At Colorado Advanced Dentistry, we know not everyone has time to wait on the phone to make appointments or request information. That’s why we make the process simple, providing convenient online access to ensure you get the help you need at a time that’s right for you.  

Our online form is extremely simple and is designed to get you in and out of our chairs in next to no time. Provide your name, phone number, email address, date of birth, and three appointment date options, and we will get back to you as soon as we can to provide details of your appointment. If none of the dates provided work with our schedule, we will contact you to discuss alternate dates.  

All fields marked with an asterisk are required for successful form submission, and we encourage as much information as possible. If you have anything else to communicate with our staff, including previous dental history, special needs regarding dental visits, or past treatments we should know about, feel free to include this in the Notes for the Doctor field. 

Reasons for your visit 

In requesting your visit, you will need to provide the reason for your visit, such as a dental exam or a dental cleaning. 

Dental exams 

In a dental exam, your dentist will examine the teeth for signs of decay, evaluate gums, and draw conclusions regarding the health of your mouth. Exams may also include X-rays and other methods of diagnosis.  

Dental cleaning 

A dental cleaning is a professional approach to cleaning the teeth. Instead of using a simple brush and toothpaste, dental hygienists use specialized tools to clean all areas of the teeth, including those challenging to reach from home. We will also polish your teeth and apply fluoride treatments to strengthen tooth enamel and help prevent cavities.
